Step By Step Ordering Procedure

STEP 1: Provide Personal Information
Simply provide us by phone, fax, mail or online, the required contact information including your First Name, Last Name, Phone number, and Email address. We will contact you for further information and details.
STEP 2: Send Forms
Print and fill out Forms, then fax them to us along with a photocopy of your prescription.
What order forms are required?
Form A: Authorization, Limited Power of Attorney and Release Form (Complete on first order)
Form B: Patient Allergy / Interaction Form (Complete on first order or if health condition changes)
Form C: Canadian Medication Order Form (Complete every time you order) Photocopies of your Prescriptions (Must be provided)

Choosing to mail in your prescription order by standard delivery can add two to three weeks onto the estimated order processing time. A good alternative may be to send your envelope by Global Priority Mail which offers a delivery time of three to five days. The cost for this service is $4.00 and can be arranged by the U.S. Postal Service. This service is convenient for those who choose to send money orders with their orders.
If you are paying by credit card, your order can be sent by fax. Most office equipment stores such as Kinko's, Mail Boxes Etc. provide faxing services for a small fee.

Processing Your Order
Current Estimated Processing Time
Estimated processing time refers to the time from when we “receive” an order until the time it reaches your door. Currently, your order may take up to 1-3 days to process and up to 7-14 days for shipping, provided that there are no problems with your order (i.e. missing information, credit card processing difficulties, mail congestions). Please plan ahead, as we would not want you to run out of existing medication before your prescription order reaches you.
Money Saving Tips
It is generally less expensive to order a 90-day supply, this way you will only be charged one delivery charge for the 90-day supply rather than one for every 30-day supply ordered each 30 days.
Return Policy
As a result of applicable laws prescription medication can not be returned.
How To Order A Refill
Please check your prescription label to ensure that there are refills left on your prescription. If so, approximately one month prior to running out of existing medication, please contact our Order Processing Center at 1-866-335-8064 to place your refill order.

You will reach a customer Service Representative who will require the following information: your name, phone number, name of the prescription to be refilled, credit card number and any address changes.
